Induction Chemotherapy
(One Chemotherapy cycle consists of around 21 days)
Induction Chemotherapy Cycle 1
Day 1 to 5
Cyclophosphamide (CPM) IV over 30-60 minutes
13.3 mg/kg/dose daily for five days
Topotecan (TOPO) IV over 30 minutes
1.2 mg/m2/dose daily for five days
Day 6 starts
Filgrastim (GCSF) route SubQ, Myeloid growth factor administration daily until post-nadir ANC>1500/µL.

Induction Chemotherapy Cycle 2
Day 1 to 5
Cyclophosphamide (CPM) IV over 30-60 minutes
13.3 mg/kg/dose daily for five days
Topotecan (TOPO) IV over 30 minutes
1.2 mg/m2/dose daily for five days
Day 6 starts
Filgrastim (GCSF) route SubQ, Myeloid growth factor administration daily until post-nadir ANC>1000/µL. Once post-nadir ANC>1000/µL increase dose to 10 micrograms/kg and continue once daily until PBSC harvest is complete. Discontinue at least 24 hours prior to next chemotherapy cycle.

Day 1-3
Etopside (ETOP) IV over 60 minutes
6.67 mg/kg/dose daily for three days
Day 1-4
Cisplatin (CDDP) IV over 60 minutes
1.66 mg/kg/dose daily for four days
Day 5 starts
Filgrastim (GCSF) route SubQ, Myeloid growth factor administration daily until post-nadir ANC>1500/µL.

Induction Chemotherapy Cycle 4
Day 1-2
Mesna (MESNA) IV over 15 minutes
14 mg/kg/dose for two days immediately prior to Cyclophosphamide (CPM) and again at 4 and 8 hours after Cyclophosphamide (CPM)
Cyclophosphamide (CPM) IV over 6 hours
70 mg/kg/dose daily for two days
Day 1-3
Vincristine (VCR) IV push over 1 minute or infusion via minibag
0.022 mg/kg/dose daily for three days prior to Doxorubin (DOXO)
Doxorubin (DOXO) IV over 24 hours
0.83 mg/kg/dose daily for three days
Day 4 starts
Filgrastim (GCSF) route SubQ, Myeloid growth factor administration daily until post-nadir ANC>1500/µL.

Day 1-3
Etopside (ETOP) IV over 60 minutes
6.67 mg/kg/dose for three days
Day 1-4

1.66 mg/kg/dose daily for four days
Day 5 starts
